SOCIAL WORKER/INPATIENT THERAPIST - San Diego Hospital
TITLE: SOCIAL WORKER/INPATIENT THERAPIST
DEPARTMENT: INTPATIENT SERVICES
REPORTS TO: DIRECTOR INTPATIENT SERVICES
STATUS: NON-EXEMPT PER DIEM
SUPERVISES: N/A

Position Summary: To provide quality case management and psychotherapeutic services to adult psychiatric patients and their families.

Essential Duties:

• Conducts psychosocial assessments to document history and identify preliminary issues for treatment focus by interviewing the patient and their family members.
• Provides group and family process-oriented therapies as well as conducts educational and other didactic groups for patients and family members using various professional treatment modalities.
• Develops and coordinates an individualized discharge plan for the patient by utilizing treatment team and chart information to determine the patient's aftercare needs.
• Identifies and assesses family or community resources such as, group homes, or mental health practitioners to meet patients' aftercare needs.
• Attends regular treatment team meetings to provide social work perspective to total case management of the patient by reading and discussing written progress notes from charts and communicating any state or local agency legal requirement for case management with the interdisciplinary team.
• Reports to community/state agencies any patient issues as required and represents facility in any legal proceedings.
• Provide backup services for incoming assessment and referral calls.

Qualifications: LCSW/MFT OR LICENSE ELIGIBLE; masters degree in social work or equivalent masters degree in a recognized mental health field; experience with chemical dependency and relapse prevention preferred; written and oral communications skills, skills in case management, time management, and crisis intervention common to acute psychotic as well as to non-violent crises intervention practice. Knowledge of acute adult psychological disorders; advanced principals of abnormal psychology as specifically applied to adults; familiarly with follow-up resource services available to adults; skills in conducting marital and family group therapy; skills in maintaining information as highly confidential; ability to clearly summarize pertinent clinical information via written correspondence and medical records documentation.

Physical Requirements: Ability to stoop, crouch, reach, stand, and lift up to 50 pounds.

Registered Nurse-Intake - Chicago Lakeshore Hospital
We are currently in search of Float Pool Intake RN's who will be responsible for management of inquiry/crisis calls, clinical and medical assessment and legally/clinically appropriate admissions for the hospital. This candidate will coordinate the admission process with all units/departments and provide accurate patient information to hospital staff. Will also need to coordinate transfers to and from other facilities. This candidate will oversee the provision of social service intervention, coordination, assessment and referral to patients and families. Position Summary: To provide direct patient care to geriatric, adult, adolescent, or child patients with psychiatric disorders; to ensure the safety and well-being of these patients; to support the maintenance of therapeutic milieu.

Essential Duties:
• Participate in input and follow through with regard to treatment planning
• Observe and identify patients problems, needs and strengths, and suggest treatment plan modifications
• Provide input on, and implement patient discharge plans
• Accompany and transport patients as assigned
• Facilitate patient community meetings
• Maintain patient, and unit related documentation
• Admit new patients to the unit, program and milieu
• Maintain a safe, orderly, and therapeutic environment for patients
• Assists patients with daily living activities consistent with treatment plans
• Carry out administrative and clerical assignments to support patient care, and organizational operations.

Qualifications: A minimum of 1 yr health care experience, current CPR and MAB required. Familiarity with medical and psychological terminology, basic principles of psychology and human behavior preferred; college level reading and comprehension, writing and mathematics skills; skills in analyzing and evaluating information; problem-solving skills; excellent oral communications; legible handwriting; ability to concentrate on tasks and meet deadlines; ability to maintain confidentiality; basic data entry skills preferred.

Position Summary: To supervise and deliver quality general and psychiatric nursing care to all patient populations typically 11 years of age and older hospitalized for treatment of psychiatric, emotional disorders, and addictive disease; to serve actively and constructively as a member of the multi-disciplinary treatment team, supporting the hospital philosophy of nursing care; to ensure the safety and well-being of each patient; to provide administrative support to staff during after-hour hospital operations.

Essential Duties:
• Conducts admissions in compliance with applicable standards, regulations and hospital protocol.
• Schedules the hospital's clinical staff.
• Designs and conducts training programs for clinical staff.
• Carries out the personnel management activities associated with direct staff supervision, including screening and selection, orientation training and development, performance management, and employee relations functions.
• Serves as the department's fiscal manager.
• Supports the organization's mission, goals, and objectives.
• Represents administration during assigned shift.
• Supervises hospital wide nursing care during assigned shift.
• Participates in activities, which enhance professional growth and development.
• Complies with facility safety, infection control and security program.
• Supports facility internal and external customer service standards.

QUALIFICATIONS AT ENTRY: Possesses current RN's license; demonstrates sound leadership skills and utilizes these skills in supervising clinical staff working with patients ranging from 11 years old and up. Basic knowledge of medical terminology thorough familiarity of psychopharmacy and the use of psychotropic medications within all age groups; basic teaching and training skills helpful; problem-solving; organizational and time management; crisis intervention skills; written and oral communication skills; eligible for CPR and PART certification; skills in establishing and maintaining effective working relationships in a variety of clinical and support team members; empathy for patients and their families; flexibility regarding hours and schedule worked; skills in maintaining information as highly confidential. Consistently high quality of clinical and interpersonal skills to be an exemplary role model to other staff; displays basic knowledge of treatment procedures and non-violent crises intervention practice. In the absence of these qualifications upon hire, must demonstrate ability to learn these concepts and develop these skills within reasonable time frame via the hospital's orientation and training program.
